Thank you for choosing Peak Form Medical Center
Please allow about 1 hour for your first appointment. Your physical therapist will perform a comprehensive evaluation, administer therapeutic treatment, discuss your treatment plan, and explain any necessary home exercises that will help advance your recovery.
What to Bring to Your First Appointment:
- Insurance card
- Driver’s license or other picture ID
- Doctor’s prescription (if you have one)
- Comfortable clothing and shoes with easy access to injured area (we have changing space in the office)
- Calendar to schedule follow-up visits
- New patient paperwork (see information below)

A Personal Wellness Plan Tailored for You…
Your physical therapist will perform a thorough evaluation and develop a plan of care to address your specific areas of need. We use a variety of treatment techniques that reduce pain, improve strength and mobility, restore function, and prevent disability. In addition, your physical therapist will also develop a tailored home fitness- and wellness –oriented program to supplement your treatment plan and support continued progress outside of the clinic.
Deductible and Co-pay:
Be sure to contact our office with your insurance information prior to your first visit so our front desk may verify your physical therapy benefits. If you have a deductible or copay, please be prepared to make a payment at your first visit. We accept cash, check, Mastercard and Visa.
