What can you expect from your first appointment?

• Please arrive 30 minutes prior to the appointment time for initial paperwork, or create your patient portal account and complete paperwork online. Create your patient portal here >>
• Please allow an additional 1 hour for the first appointment. The occupational medicine provider will review the specific work-related injury/illness while also conducting a thorough medical history. A comprehensive physical examination will be performed.
• At the conclusion of the appointment the plan of care will be discussed with the employee and employer. A WC164 will be provided to the employee that lists any referrals made, any prescriptions, return appointments, and any work restrictions.
  • A WC164 is state required form for reporting work related injuries.
• As the injured employee, expect contact from the workers’ compensation adjuster by mail or by phone.
